Has space to fill out name, rank, date and confirm that the goggles have been worn for at least three months. It then questions what duties were carried out - pilot, airgunner, WOP/AG etc, hours flown, highest and longest flights and whether oxygen has been used. Finally it questions the comfort of the goggles, whether sight was improved and anti-dim was regularly used. This form is unused.
