Filled out for a Hampden bomber of No.14 Operational Training Unit, Cottesmore. This was caused by error of judgement by the pilot who anticipated that a floodlight would be used and his approach was affected when this additional assistance was not given. The aircraft's port wing hit the top of Chancelight. Signed by Group Captain F.H. Laurence and dated 1st March 1941. The paper is yellowed, folded/creased and has some edge damage. An unusual find.
