Lists the inspections required with a space to mark down the condition/notes etc. Space at the end for the aerial engineer and pilot to sign the form as completed. The latter 2 pages cover daily inspections and are laid out in a similar fashion. Stapled in the upper left corner. Staple corroded and top sheet loose. Unused but named to Lt. Doughall.
