Typed questionnaire issued by the consultant in opthalmology at the Central Medial Establishment regarding the use of 'corrected flying goggles'.  Has space to fill out the airman's details, his aircrew position, number of hours flown, highest & longest flights, oxygen use details etc.  Also asks if the goggles are comfortable, whether they mist up, if anti-dim was used and so on.  A fascinating and now probably unique document relating to the use of corrective lenses by aircrew.  Somewhat creased with some edge damage.  Stock code P18368.
